Shift Details:

• Requested Date of Coverage and Schedule:
  • November 23 - November 27
  • November 30 - December 4
• Schedule Details: Monday - Friday, 8am - 5pm onsite daily (no weekends necessary). Team conferences on Tuesday and Friday at 8am.
• Procedures: Pre‐Admission Screening Review and Documentation per CMS Guidelines, Prescribing Narcotics, Discharge Summary/Med Rec, Weekly Care Plan Documentation, Post admission evals, face to face visits, IPOCS, PAPE/H & P
• Certifications Needed: BLS
• Requirements: BC, MI License, BLS, Clean Background, Must be available within the stated dates, Must be local (no exceptions.)
• Patient Population/Case Load: 100% adult rehab; 10-20 patients per day on the unit
• Case Mix: Inpatient Rehab Unit (Only Hospital Inpatient Rehab coverage)
• Trauma Level: Level 3
• Additional Note: 24 Hour Lab Available, 24hr Anesthesia Department
• EMR in Use: Cerner

Licensing Details:

• License: An active Michigan state license is required.

• Estimated Privileging Timeline: 30 days
