Job Description
We are seeking a dedicated and compassionate Family Medicine - Geriatric Physician to join our team in Oregon. This locum tenens position offers a unique opportunity to work within a PACE program (Program of All-Inclusive Care for the Elderly), providing comprehensive care to adults over the age of 55. The ideal candidate will have geriatric experience and be comfortable with a mix of in-clinic and community/home visits.

Day to day
Is Hospital Stroke Certified?: Yes Providers will manage a combination of in-clinic patients and off-site community patients. Off-site duties require driving to patient residences (Assisted Living facilities, SNFs, or personal homes), typically accompanied by a team nurse for the visit. The provider will collaborate with an 11-person interdisciplinary team. Most patient management and care coordination is handled through the EMR.

Other details
