Day to day
Team Size: 2 providers, 3 APPs, 2 LMSW, Pharm D, Care manager and 21 support staff Enjoy the opportunity to develop meaningful relationships with patients, practice the full spectrum of family medicine, and make a lasting impact in a close-knit community. A typical day may include: Providing direct patient care Collaborating with multidisciplinary teams Participating in teaching and mentoring Documenting in Epic EMR Engaging in quality improvement initiatives Supporting community health efforts

Call
On call is shared between several offices. Physicians rotated a on call week throughout the year (approx. 8 times a year depending on the number of providers in the call pool.)
