Licensing and Privileging:
- License: An active Indiana sate license or an IMLC membership is required.

- Estimated Privileging Timeline: 60-90 days

Day to day
General Description: The practice consists of general anesthesia, OB, vascular surgery, cath lab, healthy peds, occasional NORA, and basic nerve blocks. No CRNA supervision. Support Staff 1 assistant; NO CRNA

- Shift pattern: Shift+Call
- Setting: This provider would work regular OR days Mon-Thursday, with cardiac call ONLY on the 24, 25, 26 and 27. 7a-3p; There will NOT be any night call.

Call is 7am December 24 – 7am December 28, 2026.

Licensing & requirements
- Active state license required
- Estimated privileging: 60-90
- ACLS certification
- BLS certification
- DEA controlled Substance Registration Certificate certification

cardiac fellowship trained physician required IMLC accepted Facility is ok to review a provider who are not Cardiac fellowship trained but can provide the cardiac case logs & attestation form for all the procedures.
