Graduate of an accredited Nurse Practitioner program with a Master’s degree in Nursing– [REQUIRED] Nurse Practitioner Certification by ANCC or AANPCB – [REQUIRED] Minimum 3+ years cardiac or cardiovascular surgery experience with ICU patient management or experience with endoscopic vein harvesting and first assisting - [REQUIRED] Unrestricted Drug Enforcement Administration (DEA) license to prescribe medications – [REQUIRED] Full prescriptive authority, in accordance with medical staff privileging – [REQUIRED] Unrestricted medical staff membership with appropriate practice privileges – [REQUIRED] BLS certification issued by the American Heart Association or American Red Cross – [REQUIRED] Eligible to provide reimbursable services and maintain provider status with Medicare and Medicaid – [REQUIRED] No current or prior sanctions or exclusions from HHS/OIG or the GSA List of Excluded Individuals and Entities – [REQUIRED] Permanent Placement Fee $18K (note in "Additional Submission Details" field) - [REQUIRED]
